We see patients who have had joint replacements, cardiac surgeries, weakness, balance issues, frequent falls, strokes, Parkinson’s, COPD, dementia, pain and other issues.
Our therapists work together with each other and with doctors and nurses to help our patients achieve their highest functional level so that they can live safely and independently in their homes by the end of our home health physical therapy course of treatment.
If possible, we want to help our patients regain their prior level of function – (What were you recently able to do that you aren’t able to do at the present time? Walk several blocks? Play golf/tennis? Climb stairs? Walk without a walker?)

Physical Therapy
Physical therapists can help patients: restore balance and functional mobility, fall prevention, safety assessment of patient and home environment, provide care-giver training and education, pain reduction, design home exercise programs.
JOINT REPLACEMENT PATIENTS:
Reduce pain, provide a home exercise program, help you regain function, increase range of motion, prepare you for further therapy at an outpatient physical therapy clinic.
PATIENTS WITH RECENT OR FREQUENT FALLS AND BALANCE PROBLEMS:
Falling is not a necessary part of aging. We approach your balance problems with the intention (not a guarantee) of making you fall-proof. We will work to correct specific balance problems during our treatments, and you will be given a home exercise program to work on in between our treatments.
PATIENTS LACKING STRENGTH AND ENDURANCE:
Your physical therapist will help you perform exercises and activities during your treatments to help you regain strength and endurance. You will be given a home exercise program to work on in between physical therapy treatments.
Occupational Therapy
Our Occupational Therapy team can help patients improve the following skills and activities of daily living: dressing, cooking and eating, toileting, personal hygiene, grooming, bathing, dressing etc.
They may help patients obtain specialized tools and self-care aids to help patients adapt or overcome limitations in performing these skills.
Occupational Therapists also help improve cognitive skills such as memory function, decision-making, abstract-reasoning, problem-solving and perceptual skills, sequencing, and coordination.
Another specialized area of occupational therapy pertains to the rehabilitation of the upper extremities after injury, stroke or surgeries, etc.
